Mizora is a side character in Baldur’s Gate 3 that is the definition of a literal devil. She has enslaved Wyll via a contract to do her bidding, which in turn means Wyll works for her boss, Zariel. Karlach also has some beef with Mizora, as the two were on the same team when fighting for Zariel in the Blood War. All of that can be forgotten by your character, though, as you have a legitimate chance to romance Mizora at some point in BG3.
You can find out exactly what you need to do to make this happen in the guide below. As a warning, there will be spoilers for the end of Act 2 and the start of Act 3.
Romance Mizora in Baldur’s Gate 3
The first and most important step in romancing Mizora is to save her from the mind flayer’s ship at the end of Act 2. After jumping down into the mind flayer pit atop Moonrise Towers, you will be able to find Mizora trapped inside of a pod, similar to the way you were at the start of the game.
When you find Mizora, you can choose to let the mind flayers keep her or you can break her out. If you have Wyll or Karlarch in your party, they will strongly advise against this. However, Mizora offers Wyll the chance to be released from his contract in six months, which sweetens the pot. Naturally, to romance Mizora, you need to release her from the pod.
If you do that, the next time you see her will be at the Wyrm’s Rock Fortress after Act 3 has begun. Mizora is there to watch Gortash be crowned as the archduke of Baldur’s Gate, and speaking to her will reveal that she knows where Gortash is keeping Wyll’s father, the previous archduke. It’s at this time that Mizora offers to join your camp so she can how things play out.

After Mizora joins your camp, you will be able to find and talk to her just like any other companion. Although, when doing this, Wyll will always be behind you. For me, I waited throughout several long rests before speaking to Mizora again. Upon doing this, she offered to treat me to a night of pleasures “more satisfying than mortal flesh, blood, and bone.”
When this happens you want to select all of the dialogue options that lead to a romance scene. If you do, Mizora says she will come wake you up in the middle of the night, and a romance scene ensues.
However, after this happens, Wyll will find out and confront you about it. No matter what you say, he will be extremely disappointed in your actions.
